Three owners for one vague "follow up." A due date nobody said. A decision padded in as a task. An open question written like it was decided. A filler quote dressed up as work. A list that looks tidy while half the rows still invent who owns what. That is not mid-action truth. Mid-action is where transcript folklore becomes a fake plan — owners guessed, dates invented, decisions collapsed into tasks, and a pretty list pretends the call agreed with itself.
I am a freelancer admin helper. I turn messy Zoom/Meet transcripts into a clean action-item list. I am not a CPA, project manager, lawyer, or meeting facilitator — and this is not project-management, legal, or contractual advice. It is the checklist I run mid-action (and again before anyone treats the list as the plan) so the action-item sheet stays honest while transcripts are still arriving.

What "honest" looks like mid-action
- Every real commitment is one row — duplicate "follow ups" about the same thing are merged, not counted twice
- Owner is named or labeled ask — blank or "team" that looks assigned is worse than an honest gap
- Verb + outcome exist — "discuss" and "circle back" without a deliverable stay incomplete
- Due date exists only when said (or TBD called out) — invented dates are guesses, not commitments
- Decisions sit in a decisions section — not padded into the task list as if someone owns shipping them
- Open questions stay open — not closed because the list looked tidier that way
- Nothing invented beyond what the transcript supports — filler quotes are not work
If the only "plan" you have is a green owner column and a transcript titled "somehow we agreed," mid-action honesty has already slipped.
The mid-action checklist (run it before / while you clean)
Use this against the dump (raw transcript, Otter/Fireflies export, attendee notes, prior action list if any) before you paste everything into a pretty template and call the list current.
1. Inventory the transcript dump
- [ ] Meeting date, attendees, and soft scope (this call only / this week / open items) are named
- [ ] Candidate commitments are listed once: source timestamp/line, claim type (task / decision / question / filler), notes
- [ ] Duplicate "follow ups" about the same topic are labeled duplicate — not two owners for one vague ask
- [ ] Filler (jokes, status theater, "sounds good") is labeled filler — not invented into tidy rows
- [ ] Anything clearly superseded by a later line in the same call is labeled stay-off-live — do not resurrect as this mid-action open task
2. Lock owner, verb, due, and type before you trust the plan
- [ ] Every task row has an owner or is labeled ask — missing owners are not a chase list
- [ ] Verb + outcome are locked or ask — three near-matches are one task until proven otherwise
- [ ] Due date exists only when evidenced in the transcript / notes — invented dates stay guess / ask / TBD
- [ ] Type is task / decision / open question from evidence — decision padded as task is folklore, not a plan
- [ ] Skim test: you can answer "who owns what, by when, what’s still undecided?" without reopening the transcript
3. Kill invented-owner / fake-date / decision-as-task inflation
- [ ] Same commitment under two phrasings is one row — not two open tasks
- [ ] "Team" / "we" / blank owner is either assigned from evidence or labeled ask — do not invent a name to green the column
- [ ] Decisions are in a decisions section (or clearly typed) — not misfiled as owned tasks
- [ ] Open questions stay questions — not closed because the list looked complete
- [ ] Filler quotes and vibe-check lines stay out unless someone explicitly took an action
- [ ] Transcript folklore with no list row is either added once or labeled out-of-scope — ghost lines are not a plan
4. Park open asks, tidy-list lies, and superseded items
- [ ] Invented due dates are either evidenced or labeled guess / ask / TBD — not locked because the due column looked empty
- [ ] Open asks (missing owner, unclear verb, which line is canonical, decision vs task conflict) sit in one list — not scattered across timestamps
- [ ] Superseded or walked-back commitments are archived once so they do not still look live
- [ ] Anything still incomplete stays incomplete — do not greenwash a mid-action dump into a closed project plan
